It is not possible by using default feature as prestashop not  facilitate us to create attribute of checkbox  type , though the provide facility to create attribute or radio  type,

we can make some custom code to achieve this on fronted and need to manage add  to cart accordingly.

For price we can show on the basis of prices enter in back-office for attribute  

Let me know if further any query, it can be achieved by custom code
