Can't add address

Please see the attached... when a customer is trying to add an address, all they see is optional fields.  If we try to press the save button, we get a 500 error.  We have no idea what happened but this just started.  Is there a module we can reset or re-configure?

Ok... it seems I had the default country set to Canada.  When I did that I only got the fields in the attached above.  When I changed it to Afghasistan I was able to see the whole form again.  Anyone have any idea why?  I checked the format for Canada and it looks fine.

Resolved - Apparently there were two other blank Canada countries created when we did our migration... it looks like the default was set to the latest one.  We disabled the fake countries and made sure only one was enabled.  What a waste of time.
