1.7.4.2 - Duplicate order code

I have a weird issue on my PS website, it happened already four times in a short period of time (20days) that we get weird results on order list.

The issue is that two orders get created at the same exact time up to seconds (all i can see from db/order list) but with the same order reference. 

Two happened from PayPal, one from NexiPay and one from COD (Cash On Delivery).

All of them (duplicate speaking) have different order total, and those with paypal/nexi even have the same payment transaction. I can't seem to find a way to reproduce this in my local environment and i can't spam tests on my production website. 

Are my customer little wizards or is this a PS bug?

Interesting thing is that the registered payment is the sum of the two orders minus one shipping cost for one order (as for my shop configuration above 40€ it's free shipping)
