I have the same problem but nothing is hooked to displayBackOfficeHeader. How did you solve this?
I tried replacing the file controllers/admin/AdminCarrierWizardController.php but I'm not able to fix this.
I filled the whole carrier prices but for some reason I can't do this with 3 zones.
