Changing checkout settings

Hi, I wanted to adjust a couple of checkout settings.

First, when my customers checkout the shopping cart does not require them to enter a telephone number, however I would like a phone number to be required for checkout.

second, I would like a survey which asks "how did you hear about us?" with a list of options for them to check:

1. Google search
2. facebook
3. Other: Write in box (textbox next to it)

Is it possible for me to add such a feature?

Everything is possible...

The telephone number question has been asked and answered before.

As for adding new fields for the "How did you hear about us", it really depends on how clean you want it to be, you'll need some knowledge of php + smarty to do it, you can see how existing fields are accessed / stored, and you will need to duplicate one of them (like last name) and rename it.

This includes tpl files, php files, and php class files, as well as creating a new column in the database for it.

Please don't take this the wrong way, but it would be very helpful if you could include a link to the telephone number fix in your reply. This was one of the first threads to show up when searching for the fix, and it hasn't really helped.
