How to add to Contact Us Page - Help Please

Hi all, im trying to edit the contact us page, the one with the form. I want to add my address and contact details before the form.

Iv tryed adding the follwoing code but the form just disapears and the contact page goes blank. Does anyone how or where I need to add teh code etc. Im new to this so its just trail and error. Thanks

please post your entire contact-form.tpl file here as there is an error somewhere that is causing the page to break.

Try tpl in zip file attachment.

Forum have restrictions to what type of file you an attach so safest way to zip them.

Yes it is attached correctly but I already posted changed tpl file in previous post.
It need some styles to be added in global.css but it should work.Try it.
