Module "productquery" does not work after theme installation

I have a module called productquery that adds a contact form on the page of each product, and the visitor can fill it out to ask a question that is sent directly to my email.

 

The problem is that after installing a new theme, this module stopped working. I'm using Prestashop version 1.6.1.17.

 

I would like to know how I could find the bug. I don't find any errors when inspecting by using the browser tools, and I don't get any error message after submitting the form.

Hi David, i don't know about your module, but if you may, i like to recommend you a module that does exactly what you need, is also free and is working (i downloaded your module and doesn't work in my current PS 1.6.1.17 store).
