1) You made some changes on code ?

2) Deactivate all overrides and non native modules on your back-office: Tab "advanced parameters" -> performance -> block debug mode. Set both options there to YES and save.

Clear cache of your browser and smarty. Clear the cache of smarty is on same tab named before on right top (button). Clear browser cache = button F5 on your keyboard.

Link of your site is ? I will take a look into the source code, if zip is loading or not.

Which Prestashop version ? Which php-version in use ? Which modules did you activated, there are not native Prestashop (payment, delivery/shipping, check-out) ?

1) latest screen you attached:

Your server is having some problems with cache files (if files on FTP available, than you are having permission errors). Contact the server administrator and ask for to fix.

2) For PS 1.6. you should take in consideration to use php 5.6. instead. See here tested server configuration: https://www.prestashop.com/forums/topic/633856-server-requirements-tested-in-production-for-ps-16/

3) If I try to place an order, I cannot notice nothing irregularly. I have the field ZIP as option, missing on your screen added before. So I presume that you are having a cache problem (delete  browser cache and try again). Furthermore the problem could be related to what I mentioned on 1) you should fix this with your server administrator.

You are using PS v1.6.0.14.  There have been at least 18 new versions of Prestashop released since.  You should likely consider upgrading to PS v1.6.1.17
