Order paid for but details not recorded by PS

We were recently contacted by a customer who placed an order on our shop but had received no confirmation email.

When we checked the details, the order had been paid for through PayPal but there was no record of the order in the list of orders.

The details showed up in the list of customers and the shopping cart was loaded with the ordered goods.
But it says "No order created from this cart".

How can this happen?
Has anyone else had a similar experience?
