Change contact us module URL

Hi guys,

I purchased and installed the Extended contact form module. https://addons.prestashop.com/en/contact-forms-surveys/15703-extended-contact-form.html

When I install it, it overwrites my current contact us page. 

I do not want this, I want this module to be displaying in a custom URL on the home page. 

 

Should be a case of making changes to;

contact-formXXX.php

controllers/ContactXXXController.php

themes/YourSiteTheme/contact-formXXX.tpl

 

Can anyone help?

The best approach would be to contact original module developer, because nobody can offer the solution for you so fast as the person who made the module.
