User Created Attributes?

I'm exploring using PrestaShop for a nonprofit that distributes clothes and toys to needy children through social workers. Prestashop would allow the social workers to place orders. As part of that process, we need the social workers to tell us about the kids (first name, age, gender) and answer a few other questions about their situations. Then we need the kids' names to show up as an attribute on every product - and print out on the packing slip with each item and in a separate section. The answers to the other questions need to print on the packing slip too. 

 

Anyone know if that can be done in Prestashop? Thanks for your help!

I have never used the text customization feature of PrestaShop but if you look under customization in when adding a product you have the option to add as many text boxes as you want. That should do what you want at least for the orders -- no idea what prints on the packing slips as I don't use those either
