Product personalisation addon

Hi there,

 

I'm looking for a Prestashop module which works in the same way as this Shopify addon:

I've found a few modules but they appear to be more complicated than I require - all I need is an extra couple of fields (text field, radio button etc) for each product. It would also be good if the module replaced the 'Add to Cart' button with a 'customise' button too if possible. 

 

Does anyone know a module which might be suitable?

But you can also use the default customize area of prestashop and move it to another place in your product place. You will have to find a developer to make that work, but its possible.
