Wrong customer address saved on account creation Prestashop 1.6.1

When i create account, then wrong address saved automatically in database. 

 

what i have tried for solution:

I have replaced all files/folders except config/.. files

I have disabled all modules

I have disabled all overrides

I have enabled default theme

I have disabled whole cache/recompile etc

 

Tempary solution:

I have removed all data from ps_address tables, then it solved but after some days, when ps_address has more than 100 records then it has started again to add wrong addresses.

I have checked/analyze/optimize ps_address table.

I have compared ps_address table with fresh installed prestashop 1.6.1 ps_address table but not found solution

 

Any solutions from experts... please
