Stuck with Back office Form Creation and work flow

As i am new to prestshop... Can any one help me... HIghly apprceiated.

 

1) How to create a custom module that includes a basic login form in backoffice?

2) Can any one tell me the workflow of writing the custom module with prestashop coding standards and in which folders the form  should be created in order to follow MVC ?

1) Not sure why you'd need this when the back office already provides a login page?
