Add New Address / My Addresses / Add My First address shows HTTP ERROR 500 / Blank page

Hi, i am experiencing some problems with my website's front office and here are the details. 

I tried looking at the forums, and i found nothing that has solved this one. I found some posted about this too but there are no replies.

 

If you need more details / to access my cpanel and my backoffice just to identify the error easier, i am willing to cooperate.

Prestashop version: 1.6.1.2

Prestashop theme: default-bootstrap

Hosting: Hostgator

I am using 5 step Check out

Details:

Problem #1
1. A new User who is logged in Front Office selects "Add my First Address" in the "My Account" block

2. User cant add his first address due to this error:

chulien.com is currently unable to handle this request.

Problem #2
1. An old User who is logged in Front Office selects "My Address" in the "My Account" block

2. User cant add / update a address due to blank page (shows no error) ~ i already activated the debugging mode but its still blank

notes: i think the error showed a while ago but i cant remember if it is here or the other pages. but as long as i can remember, it says something about the following:  addresscontroller / line 136 / class / controller. I am not sure.

 

Problem #3

1. An old User can order to website chulien.com without any problem if he does not change his old address.

2. An old User tried to order again. But want it sent to a different address. 
3. at the 3rd process of the check out (03. Address), user clicked the "Add a new Address" button

4. User cant add address and cant check out due to the error:

Thing i've done:

I enabled error reporting by following the link below:

I also checked the error log in my hostgator cpanel, but shows nothing. 

I also do not know how to restore to default all the files that i have edited. What i've done to return them back to original, is i remember that part where i edited, and retype how does it look before i edit.

 

Here are the topics ive seen that has no reply / not solved forum / tried but does not solve the problem. I've actually searched a lot before posting. It just happen that i already deleted my cookies, cache and history hoping that it will do something. XD

(i am not sure if i have done them right since i have no it experience, but after i tried it and figured out that it is not working, i return the file to how it is before i edited it to avoid causing more errors)
