Add custom (php/JS) page to Prestasop

Hi all,
I have to add a new page to an existing prestashop, what I have to do it's to add a new page with a custom form, I perform different actions with JS, depending on the user group if logged, and once the form complete handle the informations with php
I read doc but I didn't figure out how to do that
I would a bit of help to begin...

And you are sure that you are using Prestashop Cloud as hosting package and not self hosted at any other provider ?
