I want an option like my demo

Hello, 

 

I want user select an option from some radio button and then when they hit next button it will open a new page and user will upload their image and then next button should be payment button. Is it possible to make a system using my existing PrestaShop site? 

 

* This form will send to the admin automatically like contact form.

Is it very small work? Or need big time to complete for a developer?
