Phone Number Fields Missing In Admin Address page

Greetings!

 

Customer website using 1.6.1.7 has the customer phone number fields in the front end registration but they are not on the admin>customer>address page when you add an address to a customer.

 

I need to add the two phone number fields to this address input page, how can it be done?

 

I see solutions that pertain to prior versions but nothing that references 1.6.x.

Do you mean when you are creating a new address? Or that they should show up in the Addresses table itself?

I can see 2 phone number fields are right there on address add/editing page.

I have the same problem after upgrading from 1.6.0.9 to 1.6.1.18. 

You have to go to Admin -> Localization -> Countries and edit the address format there to include phone numbers again.
