Cart Item attributes selected do not appear in the order

Hello,

 

We have created a number of items that have multiple attributes.  An example would be a golf shirt that has attributes for size, car club and logo choice.  We have discovered that some attribute selection do not come up in the order that we see in the backend.  Size never shows up, although the fronted presents the dropdown for the customer to select it.  This is likely a configuration error on our end but I don't know where to look to see if items with multiple attributes have been set up properly.  Is there a documented guide to do this?
