CSV Import not importing sub sub categories

I am using 1.3.0.7 to import a CSV my file is attached.

The issue is sporatic some import some don't I think that I have setup the file correctly can someone please verify?

Do parent categories need to be ID's or Name's?

Also can we list the categories, sub categories, and sub sub categories in any location in the CSV as long as the ID's match the parent category?

Yes, the parent category should be the ID of the category and the order of the categories shouldn't matter. It looks you are formatting your CSV file correctly. The only issue I can see is that you have categories with the same name (for example, 47 2.0L and 52 2.0L). The category import in Prestashop doesn't allow you to have multiple categories with the same name. Perhaps using different sorting prefixes will get around this issue, but I've never actually tried it.
