Info Request Module With Thank You Page Redirect After Submit

Hi!

 

I need to track the conversions (leads) that are coming through Adwords Campaign. The site has some  products that are not sold online. For this products I have an ajax info request module, but it does not redirect after submit to a Thank You page, so I can not track the conversions.

 

I need an Info Request module which has the following specs:

1. To be hooked in the product page

2. To be able to modify the fields

3. To redirect to a Thank You page after submiting the request.

 

Does anyone know such a module?
