Rebuilt newsletter box

Hi,

I would like to need a box on the left side with a passing head. Then i need AdressInfo, a flipdown menu with three choises. A place for their postaladress is also needed.

My first thaught whas to build over the newslettermodule but i did figure it out? This will be used for the customers to fill in to get our papercatalouge sent to them, is it doable?
