Contact Form Displaying Photo-Product + Atribut

I hope get more lucky in english to rich what I looking for.

 

When I am configuring my product For example "color" + "Size" , "quantity" . and I click on my button called " budgeting by mail " what I want is that the next screen is a form showing me the attributes previously selected and I can send a mail with the "name" fields "email" and " phone". and I get an email to my mailbox . I do not need to see that on my panel prestashop mail order , but doing better.
