How We Can Take Prescription For Glasses ?

I am using Prestashop for selling glasses and sunglasses. With prescription glasses there are few normal attributes - like the color and size for example.
But I need to add a feature where customer can select from three options /no prescription, single vision, multi vision/ and if single vision or multi vision is selected I need the customer to enter their prescription /text box or select is ok - prefer text/.

Please any one has an idea how to solve this problems? or is there some module that I can show an other product with attributes. /prescription lenses/ with in the main product /frame/ page???
