Fields missing in the authentication form

Hello, I allready posted in the french forum but I try also with the english one to have more chances of answers ... But I hope my english isn't too bad ...

 

I made the update of PS 1.5 to 1.6.1.1.

And I get the multilangage on (french + english)

 

On the locale server, the authentication form is ok. With 5 steps. Country and birthday fields are ok.

 

But on the online website, these fields are missing. And the address is in the 2d step and not in the 3d.

 

Field country is also missing on the form to change address for shipping or for facturation.

 

I really need help. I copied all files of the theme from remote to locale server. But the form is always ok on the locale server.

A server problem ? But what ???

I found the problem, it's a module added to create form. I see with the developper.
