How to create insert form without add_to_cart product

Hi all, I'm newbie in prestashop

I want to create a simple support form in product.tpl page in order to customers or guest can leave their contact (name + phone number). Then I can call back later.

I created a simple table in database cs_simple_guest_info with two column "guest_name" & "guest_phone".

I still create insertGuestinfo function in product.php & simple html form in product.tpl too. But It's not working

I need some advice, or code to solved this. Thanks for all
