Contact US problem, Please Help?

Dear All,

I'm new here and tried to add an item to contact form as "Paid Confirmation" from admin site. Then I occurred a problem when I click the contact menu from main site (http://mysite/contact-form.php) then it blanks. There is no any contact menu form appear there.

Please help my problem and sorry my English is not good
