How to add custom product attribute from form

Hi,

 

I'm trying to develop a module to add product customizable properties depending on the name and birthdate of the customer.

 

I developed a custom theme so the customer can write his name and birthdate in the Homepage. The submit action is the URL of the product (there is only one product in the webpage) and I need to display different data in the product page and save the order with that data (it has to appear in the shopping cart and in the backend orders) depending on what the customer wrote in the Homepage form.

 

I'm trying to achieve it creating customizable properties for name and birthday in the backend, but I'm stuck here, don't know how to do it.

 

Can I have some help please?
