I would like to keep left the product description and right to replace the basket by contact form. in the modal window.

Ok, normal contact form then? It might be easier or trickier depending on what you're trying to achieve. If you're happy with having hardcoded values, you can simply add your own html form that posts to the contact-form page, otherwise you should really add some variables to the blockcart module.
The file you want to edit it blockcart.tpl, the modal window is there. Are you a programmer?

No I'm not a programmer. I counted insert a form in html format and call a php page for processing.
