How to add Logos and Printing Setup Costs? Help needed!

Hi All,

I wish to do something in PS but I can't figure out how to do it. Any suggestions would be gratefully accepted!

I sell rugby equipment including shirts:
- These shirts can be ordered with optional club/sponsor logos (between 0 and 4 on chest, left breast, right arm or left arm) and player numbers.
- Each logo or number adds 3 Euros to price of the shirt.
- There is a setup fee of 30 Euros for each logo for the whole order (no setup fee for the player numbers)

What I need to do is:
- Allow a customer to select a shirt type for their team.
- Then, they must be able to enter required sizes with appropriate player number (e.g. player number "1", size "XXL; player number "2", "XL"; etc... - note the numbers are free-form, not necessarily consecutive numbering).
- Then they must be able to enter logo positions and upload logos.
- Of course, the cost of the order needs to change based on the choices made.

So, does anyone have any clue how I need to approach this?
