Contact form in order-opc?

I would like a contact form on a prestashop store in a pop-up window that appears when customer tries to exit the page. I already have a working way of the "pop-up". But I'd like to have a form in the pop-up where the visitor can send a message to an support e-mail address.

 

I bought a great php-form, but forgot that Prestashop doesn't allow certain php because of the smart (if I remember it correctly). So how can I do this? :/

Assuming the form doesn't interact with prestashop in any fashion and just sends you an email, you could just leave it as a seperate php file and call it in via an iframe into the popup
