Botão não cria conta

Pessoal, faz alguns dias que quebro a cabeça e não consigo resolver meu problema, pesquisei na internet e deram algumas soluções, nenhuma funcionou, então resolvi tentar no forum. vamos la:

ao tentar criar uma nova conta, o botão criar (submit) não faz nada, se apertar enter ele vai, se clicar em criar conta ele não faz nada, segue meu authentication.tpl

tentei ativar o modo debug conforme instruções mas na raiz do site nao gera nenhum novo arquivo...

Erro bobo que nem percebi, veio no tema
