Need Custom Text Field on PS 1.5.6

Hey guys!  I have my website functioning OK, but I need some assistance!

 

I need custom text entry fields as an "attribute".

 

I have added the text field in the "product customization" section, but I need it to be listed as an attribute!

(higher on the page, within other attributes)

 

Unfortunately, the text box currently resides at the bottom in a tab!!!!

I need the attributes to look like this:

 

Symbol 1

Text Field

Symbol 2

Text Field

Symbol 3

I can't seem to figure out how to make the text field appear higher on the page within the attributes.
