Product combinations through CSV

Hi,

I have going to upload product combination through csv but I can't make it correct format .

Please help me to make it correct.

 

My combination format like that 

 

Product Id:538

Color - Green, Pattern - Self Design, Size - 38, Sleeve - Full Sleeve ,Quantity - 10

Color - Green, Pattern - Self Design, Size - 40, Sleeve - Full Sleeve ,Quantity - 5

Color - Green, Pattern - Self Design, Size - 42, Sleeve - Full Sleeve ,Quantity - 4

Color - Green, Pattern - Self Design, Size - 44, Sleeve - Full Sleeve ,Quantity - 6

Color - Green, Pattern - Self Design, Size - 45, Sleeve - Full Sleeve ,Quantity - 1

 

Please make a CSV file in this combination.
