HOW TO ADD A CONTACT FORM ON CMS PAGE ?

Hello People I would appreciate if any one knew how to add a custom contact form with custom fields Like

 

Name

Telephone Number

Company

Website 

City

State

Purchase Type

the problem with default prestashop contact form is that it doesn't allow to add custom fields

and even if we follow the form tutorials its too difficult to follow .

 

i knw its not that difficult to add third party HTML contact forms

 

But i was hoping to keep our customer information safe and thus keeping the information secure on our server .

 

 

so Please if any one can help i would really really be grateful Thank you
