How did you hear about us module

I have been trawling through the internet for days now looking for a module that will ask the customer when they sign up for an account where they heard about us. It seems like such a simple function of an e-commerce website but i cannot find a module that solves this or even a step by step guide as to how I could create this myself.

 

Has anyone else had this issue you could help me with?
