Urgent! Modules for Lucky Draw application

We would like to offer a job for design a Lucky Draw application :

 

the Application Page should have following :

 

1) Banner for this luck draw artwork

2) Text for rules of luck draw

3) Application form including Name, email, Contact number, product name.

4) Applicant need to upload purchases receipt image(JPG)

5) Auto reply the applicant for successful join the lucky draw

6) Auto email to us each application

7) support Chinese and English

This needs to be integrated in Prestashop? Because it seems more suitable to Wordpress with some plugin form.
