FATAL ERROR after paypal payment completion

Following is the error i get after I complete the payment using paypal, the payment goes through but no order is generated, I hav tried all the possible solutions recomended but can't get it right.

 

I have installed the latest 3.7.2 paypal module .... removed and reinstalled it again twice but no good have come.

 

Another thing I have noticed when i create manual order i see date on invoice as  Order Date:

0303-0808-2014201420142014

 

Which I have no idea why and how to fix it.
