I created fake user account. Then little test it.Here screens. At 1 screen i clicked 'add new adress' and then shown 2 screen. But where is fields like at 3 screen? Which module is responsible for this? How configure it?

this is not module

this is native prestashop feature

 

you can manage address format under localization > countries tab

click edit button near each active country

you will see there form to manage address format (you can add / remove fields from there)

I do not know what I did but when I click edit button then i see absolutly empty page. No errors, no messages. Just empty page.

please turn on error reporting

then instead of blank page you will see error report (i hope so)

prestashop above v1.5.3
open: config/defines.inc.php
 
find this line: define('_PS_MODE_DEV_', false);
change to look like this: define('_PS_MODE_DEV_', true);
