Thickbox does not load image. Just the animation.gif

I have just updated to 1.6 and the issue is that the image onclick does not load the thickbox

 

I have no errors on my console and the only error on Google chrome Inspect element console is 

"Uncaught ReferenceError: utilityFn is not defined" whic is unrelated to the thickbox

 

I have included thickbox.tpl . thickbox-modified.js and thickbox.css all on the page.

 

The box opens up but no image is loaded.

 

 

Any help would be great.
