need to add new form after address while checking out

hi,

I am looking for pretty simple solution. I just need to add one more form while checking out before address. Its a simple form where I take measurement from user. 

 

I know I need to create new controller and all. But I am stucked in integration part while checking out. I know its simple step I am missing out somewhere.

 

any links or tutorial is welcome. Thanks in advance
