Module was not accepted without a reason

i have correct all error , and recommandation when decline the module , but after all submition the same response :

And when i see it in validator that done that 
 

 

for the secure , i have forget to put index.php in the img folder but not a raison to decline it 

and for the other also not raison i suppose ?

i have now no error in security , i have resubmit the module to addons
