Popup subscription / sorting visitors' flow / landing page

I am in a great need of your advise...

 

I need to sort my shop visitors almost from the very beginning... since it's going to be an up-selling point in addition to brick-&-mortar shop which generates a significant customer flow physically mainly because it is located in a very special place. So it is kind of creating a Club for the ones who has been in that shop. For all others it should be also possible to shop though.

 

Initially I thought of creating almost a static landing page with various information, pictures, stories etc. and subscription form leading to the shop. However, I found it too complicated as I have to find the way how to transfer all visitor's data to the PS and assign to the right visitor group. Therefore, I reduced my ambitions and I now simply need an addon, which would:

 

- pop-up from time to time asking a question and if answer is positive would navigate to subscription form (preferably in the same popup),

- The form would gather the following: name, family name, email, country, date of visiting brick-&-mortar shop (and not more),

- customer filling such form would be assigned to a particular customer group in a BO of my shop (essentially to customer group but with more privileges), also having an access to exclusive items, special prices, etc.

- if answer to the pop-up is negative the visitor can still browse and shop on regular prices and items...

 

- It would be also nice to have an automatically generated sort of certificate which would be sent to the ones who have registered as club members. The certificate should include name, family name and date (from the gathering form). All other design details should be editable.

 

I would be highly grateful for any kind of advise. On the other hand, if there is nothing available like that at the moment - I am ready to consider a remuneration for such development.
