Form Builder with Payment

Hi,

I am a freelance writer assisting students.

 

On my website I need a CMS page that has a form. This form should have input fields like text, number of pages and deadlines etc. I provide assistance to students and when they want something, this form should be the one they should use to order.

Some of the basic fields are given below but I should be able to add more fields accordingly.

Email Address: (Email Address Field)
Topic: (Text Field)
Course Name and Number: (Text Field)
Type of Document: (Drop Down List)
Number of Pages: (Drop Down List)
Citation Format: (Drop Down List)
Number of Sources: (Digit Field)
Deadline: (Calender Date)
Upload Files: (Upload Files Field)

Calculated Amount: (Calculated Amount Based on Number of Pages and Deadline)(I will provide the formula or should be able to input in the settings according to my needs)

Pay that Amount: (Carry Forward the Calculated Amount to the Cart so that it can be paid through Stores Default Method Like PayPal, 2CO etc., whatever is being used for the store).

Generation of Emails to the customer and the store owner about the creation of the order and details of that order only after the payment has been paid.
