How to get UPS Ground to display in checkout?

I'm running 1.5.6.2 and have my store getting close to kickoff.  One issue is that even though I seem to have UPS running and installed - UPS Ground will not appear in checkout?  UPS next day air and UPS second day air appears but not Ground.  Ground is enabled and is  the default shipping method but nada!!?  I've been searching for an answer to no avail.

 

Any ideas on how to get Ground to show in checkout or what I must set differently?
