Forget password not working can someone take a peak for me

I just can't figure out why the forgot my password is not working on my site anymore.   If someone could take a look at it i would really apreciate the help.

I had a developer move my forget password page and add it to my authentication page. And it worked till i just updated the site.   I didn't change anything in the template files its got to be a change in the core somewhere.  This developer was good about not making the site upgrade friendly
