Database error

Hello,

 

I've downloaded a paid module which is meant to create a form, post it to my site and store the variables to a database in the back end after the user has filled it in, however the module does not create a form at all, and just does nothing after it says "loading".

 

I know practically nothing about PrestaShop, however when I click reset on the module, the attached screen shot is the page that is loaded.

 

I think the issue has something to do with it.
