Hi all let me introduce myself my name is savior in a couple of years that I play with prestashop, never had any problems, but today I wanted to create a new page hacking and created a category cms before and within a page and thus far all ok. 

my question and how can I insert a form in the page created? or because you can write html code and more but I want to insert a form as I do? thank you to all

You would have to create a new tpl page for the form and you would have to create a php page to go along with the template page that handled all of the programming.

so, basically you want to add <form> and <input> etc. tags to CMS page contents?

for what purposes you want to add this form? it's a contact form? or something similar?

you can do it with default tinymce rich text editor,

you can insert videos there and also other stuff like product images etc.
