Create custom contact form

Hello, I want to add a custom contact form to a CMS page so I can add it to the top horizontal menu. Just a simple contact form with name, email and text. Can anyone help me out?

in this case it's necessary to add form to page content (in tiny rich editor)

then, in cmsController inside init() function - add a php code to handle this form and to send email to you.

there is no other way
