How to create new contact form? I need two different contact pages

Hi

 

My website offers 3 different kind of services. For now, I'm using the regular contact form (with 3 subjects), that sends the messages of costumers straight to Prestashop. Anyway, would be significant better if, instead of 3 subjects in the same contact form, I'd have 3 (at least 2) different pages and forms cause the services I'm offering isn't related at all.

I found a great free contact module but, unfortunately, it doesn't have the option of attach a file, and most of my costumers need to send files for me.

Is this possible?
a- Two (or three) different contact forms and pages at the same website
b- All of them able to attach/send files
c- Connected with Prestashop (to view the messages directly from PS, instead of mail box).

One more little thing: How can I add customized fields to a contact form?

 

Any help would be greatly appreciated
